LIABILITITY WAIVER

I/we realize that participation in dance and fitness classes require physical exertion which may be strenuous and may cause physical injury. Despite precautions being taken by the studio, accidents and injuries may occur.  I understand that it is my responsibility to consult a physician prior to and regarding my participation in the class. By signing this form, I/we (the dancer and parent/guardian) assume all risks related to injury and the use of any and all spaces used by Dance By Mallory

I/we agree to release from responsibility Dance By Mallory, including all teachers, dancers, staff members, and facilities used from any cause of action, claims, or demands now and in the future. I/we will not hold Dance By Mallory liable for any personal injury including scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes

Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Dance By Mallory

In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the instructor or staff member as soon as possible.

I/we (the dancer and parent/guardian) give permission for photographs/videos of me/us to be used for publicity and advertising in the studio, on the Dance By Mallory media pages, and in the community.

RULES AND REGULATIONS

Please be ready for class to start – including hair, clothing, and shoes. (There are restrooms/changing area available for use.) All backpacks, bags, shoes, jackets, etc. need to be placed on shelves. You may bring a snack and/or water as long as you clean up after yourself and throw away all trash. (There are water fountains available for use.)

If you are not in a dance class that is currently in progress, you must quietly sit out of the dance area that is being used for instruction. No running or horseplay in the dance studio.

All payments are due at the beginning of each month.

You are responsible for any missed classes. No refunds will be given for missed classes. You are welcome to attend any other class of your level to make up for the missed class.

No one is allowed in the auditorium/stage area without permission. You must remain in the dance studio area.
